Custom Online Form Request
There is a new mailmerge field that may be added to documents to request forms to be completed by the patient.
It is not listed in the mailmerge fields when creating document templates.
The new field is ~online:xxxx~ where x can be a combination of the following...
m = medical history form
v = covid screening form
f = FP17PR
c = consent form
d = take deposit
p = contact details - phone number & email addres
The FP17PR form will only be requested for NHS registered patients
Examples
If you would like to ask the patient to complete their medical history form, the covid screening form and FP17PR you would enter the field ~online:mvf~
If you only want to ask for the medical history form, the field would be ~online:m~
You can only add one ~online:xxx~ field to a document
Using ~online:m~ instead of ~remote_mh~ will send the existing patient medical form to the central server for updating.
